Sample invoice, filled in

This sample is for a made-up dog walker in Ontario who is registered for GST/HST. The number follows the CRA's example format.

Invoice 1042

From: Northside Dog Walking, 12 Elm St, Guelph, ON. GST/HST number 123456789 RT0001

Bill to: Jordan Lee, 45 Oak Ave, Guelph, ON

Invoice date: November 30, 2026. Payment due: December 14, 2026

DescriptionQtyRateAmount
Weekday dog walks, 30 minutes12$28.00$336.00
Weekend dog walks, 30 minutes2$35.00$70.00

How to pay: e-Transfer to pay@northsidedogs.example

The tax is $406.00 × 13% = $52.78, for a total of $458.78. The sample shows one HST amount, because the CRA says not to show the federal and provincial parts of the HST separately (Guide RC4022, checked October 31, 2026).

Questions

Is this invoice template free?

Yes. Download the PDF and print as many copies as you need. You do not need an account or an email address.

Can I fill in this invoice form on my computer?

The PDF is a blank sheet to print and fill in by hand. To type an invoice and let the page do the math, use the free invoice generator.

What is the difference between this template and the invoice generator?

This template is a blank form to print or keep. The invoice generator is a form on our site: you type your details, it works out the tax, and you download a finished PDF.

What should go in the GST/HST number field?

Your full number, such as 123456789 RT0001, if you are registered. If you are not registered, leave the number and the tax lines blank, because you do not charge GST/HST.

Should I split Ontario HST into 5% and 8% on an invoice?

No. The CRA says not to show the federal and provincial parts of the HST separately. Show one HST amount or rate.

What rate goes in the GST/HST line?

It depends on the place of supply, which is not always where your business is. See GST or HST for the rate by province.

How do I number my invoices?

Use any system that gives each invoice its own number, and keep a copy of each one. Numbering in order, such as 1001, 1002 and 1003, is the simplest way.
